How to Apply for SSS UMID Card Replacemen

Having the SSS Unified Multipurpose ID (UMID) alone gives you a big advantage among other cards that any government office provides. The SSS UMID can serve as your valid ID and is being accepted on almost all establishment in the Philippines like in airports, banks, and commercial establishments. Losing it will mean headache  on what to present on your transactions that will need a valid ID especially if you do not have other Valid ID like Passport, PRC Card, Driver’s license and others. In this article, we will cover how to apply for SSS UMID Card replacement and tips on how to easily apply and receive it.

How to Apply for SSS UMID Replacement?

Before going to an SSS Branch to apply for your SSS UMID Replacement, check first Items 1 to 5 below:

1. Check if the SSS Branch that you intend to go has a UMID Card Enrollment Facility

Before going to an SSS Branch, make sure that the SSS Branch that you are going to has an ID Capture Facility because not all SSS Branch has that facility.

You may first check the list of SSS Branch that has SSS UMID Card Enrollment Facility here or email the SSS Contact Email at sssonlineserviceassistance@sss.gov.ph or member_relations@sss.gov.ph.

2. Bring a Valid ID or any supporting documents required by SSS

The supporting documents you will present will just prove your identity and will support SSS representative’s verification that you are applying for yourself and not of other people.

Here’s the list of the Valid ID and Supporting Documents that the SSS accepts:

One (1) PRIMARY ID CARDS and/or documents

  • Passport
  • Driver’s License
  • PRC Card
  • Seaman’s Book (Seafarer’s Identification and Record Book)

In the absence of the above Primary ID cards and/or documents, any two (2) of the following Secondary ID cards/documents, both of which should bear the correct name of the person, and at least one (1) with the correct date of birth:

  1. Postal ID
  2. School or Company ID
  3. TIN Card
  4. Membership Card issued by Private Companies
  5. OWWA Card
  6. Senior Citizen card
  7. Voter’s ID card/Affidavit/Certificate of Registration with COMELEC
  8. Alien Certificate of Registration
  9. ATM card with cardholder’s name; or bank certification if without cardholder’s name
  10. Birth Certificate
  11. Bank Account Passbook
  12. Baptismal Certificate
  13. Credit card
  14. Firearm License Card issued by the PNP
  15. Fishworker’s License issued by BFAR
  16. GSIS Card/Member’s Record/Certificate of Membership
  17. Health or Medical Card
  18. Pag-IBIG Member’s Data Form
  19. ID Card issued by LGUs (e.g., Barangay/Municipality/City)
  20. ID Card issued by professional associations recognized by the PRC
  21. Life Insurance Policy
  22. Marriage Contract/Marriage Certificate
  23. NBI Clearance
  24. PhilHealth Member’s Data Record
  25. Police Clearance
  26. Seafarer’s Registration Certificate issued by POEA
  27. Student Permit issued by the LTO
  28. Transcript of School Records
  29. Certificate from National Commission on Indigenous Peoples or Office of Muslim Affairs
  30. Certificate of Licensure/Qualification documents from MARINA
  31. Certificate of Naturalization from BI
  32. Birth/Baptismal Certificate of child/children or its equivalent

3. Prepare an Affidavit of Loss

Here is an Affidavit of Loss Template which you may print and Fill up with your details. The Affidavit of Loss should be notarized. There is usually a Notary Public located near the SSS Branch but rates may vary. If you already have an Affidavit of Loss printed, Notary fee is cheaper. It is advisable that you print and notarized the Affidavit of Loss before going to an SSS Branch.

4. Download the SSS UMID Card Replacement Form and Fill up using the Blank Ink Pen

Make sure to write legibly and correctly. Make sure to fill in all fields applicable. You may also get the SSS UMID Card Application form in the SSS Branch where you are applying for an SSS UMID Card Replacement. Fill up the Form using the Blank Ink Ballpen.

5. Prepare 200 pesos for the UMID Card Replacement Fee

Before, the UMID Replacement Card fee costs P300 but was reduced to P200 as of January 2018.

6. Go to an SSS Branch near you that has a UMID Card Enrollment Facility

It is advisable that you go very early or very late to avoid long lines and make sure to bring complete requirements for ease of transaction.

7. Accomplish SSS R-6 Form and Pay for the UMID Replacement Fee

If you already have the requirements stated above, upon arrival in the SSS Branch. Get the SSS R-6 Miscellaneous Form, accomplish it in 4 copies (it has 4 colors: white, blue, pink and yellow). Check the ID Social Security Card checkbox and put the 200.00 amount in words as the amount you will be paying for the replacement fee.

You may already get the Queue Number in SSS Payment so that you are already lining up for payment while filling up the form. When your Queue No is called, just present the SSS R-6 Form and pay the replacement Fee.

8. Submit your SSS UMID Card Application Requirements

After paying your UMID Card replacement fee, go the Queue No counter ang get another Queue number for the UMID Card Application. Wait for your Queue number to be called then submit your application requirements. That includes:

  • Filled up SSS UMID Card Application Form
  • Validated R-6 Form
  • Valid ID or Supporting Documents
  • Notarized Affidavit of Loss

After validation, the SSS Representative will direct you to line up in the ID Capture Section.

9. Proceed to Biometrics and ID Capturing Section

There will be an SSS Staff that will verify your information and will get your thumb and index biometrics. You will also be asked to sign your signature in a digital tablet then your ID will be captured.

SSS Observes the ID Capturing ICAO standard so make sure that you adhere to the following before you get your ID Captured:

  • No wearing of Contact Lenses
  • No wearing of sleeveless
  • No wearing of makeup on male
  • No wearing of sando for male
  • No wearing of eye glasses
  • No wearing of headband\
  • No waring of earrings and necklace
  • Ears must be visible
  • Client should close their mouth

10. Receive and keep the Acknowledgement Slip

After the Biometric and ID Capturing Step, you are good to go. The SSS Personnel will give you the Acknowledgement slip of your /application and an instruction on how and when to get your SSS UMID. The SSS UMID will be mailed to your mailing address. If you are not able to receive it personally, prepare an Authorization Letter together with a photocopy of your Valid ID as proof of identificaion when the delivery of your UMID has arrived.

When will my SSS UMID arrive?

Before, it took around 3-6 months before the SSS UMID Card gets delivered. But it has improved to 1-2weeks in Metro Manila, and 3-4 weeks in Provincial as of January 2018, as experienced by  other SSS Members also who have applied for their SSS UMID Replacement.
