How to Download SSS Forms Online

How to Download SSS Form Online

1. Open your browser. Make sure that your device is connected to an Internet Connection. Type www.sss.gov.ph on your browser’s address bar.
2. The SSS Website will be displayed. Click the sss.gov.ph menu (second from the left).
   
3. The SSS Website’s content will be displayed. Scroll on the lower part of the page then click the Downloadable Forms button (highlighted in green).
   
4. The page will display a Comprehensive List of SSS Downloadable forms grouped in Employer or Member forms and organized by the types of Transaction (e.g Benefits, Loans). Browse thru the Form that you need. Click the Link of the form that you desired then it will be opened in a new browser tab.
   
5. Double check the form if you clicked the right link. SSS really has separate form for different transaction and the number of forms that they hav in their website (or even in their branch) are really numerous. Download and Print in a clean bond paper. Most forms are in Long Bond Paper size. Print an extra copy just in case you have a correction. Accomplish all in Capital Letters and use Blank Only.
6. You may read the bottom part of the Form as it also contains Requirements like ID, Documents you need to present and the qualification for the SSS Benefits that you will be availing (if applicable).
