If you have properly registered as a Self Employed, OFW or Non-Working Spouse before paying your SSS Contribution, go to the SSS Branch and submit your RS1, OW1, NW1 or E4 that signifies your correct membership status registration in SSS. Bring a copy of your valid ID and present your proof in the assigned SSS Representative to correct your SSS records.
If you have not properly registered, accomplish and notarize the Affidavit of Source of Income to signify that you are self-employed and has a source of income, together with the SSS Data Change Form or E-4. After submitting these documents in the SSS Branch nearest you, wait for at least 2 weeks before your data becomes changed. Need not to worry because the SSS Contributions you paid before you have a Date of Coverage will not be invalidated nor refunded.
