Here are the options on How to Verify and Follow up the Status of your SSS UMID Application:
1. Call SSS Hotline
SSS Trunkline number is (632) 920-6401. SSS Call Center numbers are 920-6446 to 55 and is operating 24/5 which is open 24 hours every weekday. If you happen to be discouraged calling on daytime because of busy lines, you may call them at night when you are already at home.
SSS Call Operators may ask for your verification information so make sure to ready to provide it like your SS Number, Full Name, Date of UMID Application, etc.
2. Email SSS ID Management Department
For UMID related concerns, you may send an email to the Identity Management Department of the Philippine Social Security System (SSS) at idcpd@sss.gov.ph.
The SSS Identity Management Department usually respond within 2 to 3 working days emailing you the date your UMID was posted, the Philpost tracking number and your Common Reference Number (CRN), which you can use as references if you want to claim it at your local post office. You can also use the tracking number at PhilPost website to track the shipping history of your UMID.
Visiting your local Post Office may be the best and fastest way in claiming your UMID instead of waiting for it to get delivered.

- Complete Name
- SSS NUmber
- Date of UMID Application
3. Visit any SSS Branch near you
If you have the luxury of time, you my go any nearest SSS Branch near you to verify the status of your UMID Application.
You will be able to ask an SSS Representative for the real status and any cause of delay, if there is any, and any action to do get your UMID.
A feedback is given by SSS Inquiries contributor that it took two weeks to deliver his UMID from the date of his application. Their home address is in Metro Manila and was able to quickly be identified by courier.
How to Apply for UMID?
Most of us want to have a UMID Card because it is a valid identification card to many establishments. But do you know that it is a powerful 4 in 1 card for your SSS, GSIS, PagIBIG, PhilHealth transactions? The new UMID Card can also be used as an ATM to withdraw your SSS Loan and Benefits.
The Unified Multi Purpose Card (UMID) is the Unified ID Card of SSS, GSIS, PhilHealth and PagIBIG. The new UMID being released by the SSS (as of April 2017) uses a contact less smartchip technology that stores member’s SSS information and biometrics data such as fingerprints, facial image and signature.
Government employees can apply for a UMID at the GSIS Enrollment Center, while private sector employees, self-employed, and OFWs members of SSS can apply for a UMID at any SSS Branch that has ID Capture facility.
To apply for a UMID, you need to have the following requirements:
For First Time Application
- Member has no existing UMID Cards issued by GSIS or PhilHealth.
- Member has at least one (1) month contribution and a valid Date of Coverage;
- Member Information (SS Number, Name (First Name, Middle Name, Last Name), Date of Birth, Gender) must be the same in the UMID Card Application and SSS Membership database.
- Valid Identification Card Photocopy (Bring the original card/document for verification). Click here to check the list of documents honored by SSS.
For UMID Card Replacement
- Replacement fee amounting to Php 200. The same fee shall apply for the following.
- Correction/Change of name (e.g., from maiden name to married name)
- Correction of date/place of birth and/or sex
- Replacement of lost/damaged card
- Change of home address
- Change of authenticating finger due to amputation/sickness (e.g., index to middle finger)
- Update of member’s photo
- Change of old card to new card
- Change of specimen signature printed on the card
- For lost or non-received cards, the submission of a duly-notarized affidavit of non-receipt of card, or notarized affidavit of loss is required.
- For card replacement caused by change or correction of member information, the old SS ID/UMID Card must be surrendered.
STEP BY STEP PROCESS
Visit the nearest SSS Branch that has UMID enrollment/ ID Capture facility. Not all SSS Branches have the Data Capture facility. Their big branches have one for sure. Better contact first the SSS to know if the SSS Branch near you has an ID Capture facility.
- Fill up the Multi Purpose ID (UMID) Card Application Form. You may obtain the UMID Form application here. Make sure to fill up the form using a black ink.
- Bring one of the primary valid ID’s. (Passport, Driver’s License, Professional Regulation Commission (PRC) Card, Seaman’s Book (Seafarer’s Identification and Record Book). If you do not have one, bring two of your secondary identification card/documents. You may check the list of documents being honored by SSS here.
- Double check and verify the form before submitting to the verification counter.
- After verification, you will be instructed to proceed to the ID capture booth, fingerprint scanning and electronic signature process.

- Visit any SSS Branch near you, or SSS Kiosks available in some Malls, to activate your UMID Card. You can then use your UMID card to inquire your SSS Information like Eligibility to benefits, Contribution, Loans thru SSS Kiosk.
According to the published article on the
SSS Website, “SSS does not have rush application for UMID cards nor is it collecting fees for such purposes as card applications are processed on a first-in first-out basis. Recipients of these fraudulent text messages are advised to immediately report it to the nearest SSS branch or call the SSS hotline at 920-6446 to 55. They are also encouraged to file a report with their own mobile phone service provider to stop the dissemination of bogus SSS text messages through their networks.”
